Степень отрицательного воздействия ионизирующего излучения (радиации) на живые организмы зависит от комплекса физических и биологических факторов. Ключевыми из них являются следующие:
Поглощенная доза и мощность дозы. Это один из главных факторов. Поглощенная доза — это количество энергии излучения, поглощенное единицей массы вещества, измеряется в греях (Гр). Чем выше доза, тем сильнее повреждение. Однако важна и мощность дозы, то есть скорость получения этой дозы. Однократное получение большой дозы (острое облучение) значительно опаснее, чем та же доза, полученная малыми порциями в течение длительного времени (хроническое облучение). Это связано с тем, что при низких мощностях дозы в организме успевают частично сработать механизмы восстановления (репарации) поврежденных клеток.
Вид ионизирующего излучения. Разные виды излучения (альфа-, бета-, гамма-излучение, нейтроны) обладают разной биологической эффективностью при одинаковой поглощенной дозе. Это обусловлено их различной проникающей способностью и плотностью ионизации, которую они создают в тканях. Например, альфа-частицы, обладая низкой проникающей способностью, создают очень плотную ионизацию и наносят тяжелые повреждения клеткам, поэтому они особенно опасны при попадании внутрь организма. Для учета этих различий введена эквивалентная доза ($H$), измеряемая в зивертах (Зв). Она рассчитывается как произведение поглощенной дозы ($D$) на взвешивающий коэффициент для данного вида излучения ($w_R$): $H = D \cdot w_R$. Для альфа-частиц $w_R=20$, тогда как для гамма- и бета-излучения $w_R=1$.
Радиочувствительность органов и тканей. Различные органы и ткани организма обладают разной чувствительностью к облучению. Наиболее уязвимы ткани, в которых происходит интенсивное деление клеток: красный костный мозг (кроветворная система), эпителий желудочно-кишечного тракта, половые железы, хрусталик глаза, а также эмбриональные ткани. Мышечная, костная и нервная ткани, состоящие из медленно делящихся или неделящихся клеток, более устойчивы к радиации. Для оценки общего риска для здоровья всего организма с учетом разной чувствительности органов используется эффективная доза, которая также измеряется в зивертах.
Способ облучения (внешнее или внутреннее). Облучение может быть внешним, когда источник радиации находится вне организма, и внутренним, когда радиоактивные вещества проникают внутрь с воздухом, пищей, водой или через повреждения кожи. Внутреннее облучение часто представляет большую опасность, так как некоторые радионуклиды могут избирательно накапливаться в определенных органах (например, йод-131 в щитовидной железе, стронций-90 в костях) и длительное время облучать их изнутри.
Индивидуальные особенности организма. Реакция на облучение также зависит от индивидуальных факторов. К ним относятся: возраст (дети, эмбрионы и плоды гораздо более чувствительны к радиации, чем взрослые, из-за высокой скорости деления клеток); состояние здоровья (ослабленный организм, наличие сопутствующих заболеваний могут усугубить последствия); пол; а также генетическая предрасположенность (эффективность систем репарации ДНК, заложенная в генах, может отличаться у разных людей).
